Scheduling Appointments:

Scheduling appointments in our office is simple. All you need to do is call our office at 435.750.0366 and our staff will assist you in setting up your appointment. Most first time appointments will be scheduled with our Physician Assistant, Jamie Larsen. Jamie is part of our triage team and has been personally trained for your care by Dr. Young. If you are coming from out of state or out of country, advise us for more time effective scheduling if possible.

Telephone Hours: Monday - Thursday 9am to 12pm and 1:30pm - 4pm MST
           
Payment Options:

We accept most credit cards or cash. We request payment arrangements for all office services at time of visit. We also have interest free in-house payment plans available, all payment plans must be arranged in advance. Insurance companies require a co-pay at the time of the visit. All Medicare payments have a 20% co-pay.
